DOMAIN: INDEPENDENT FUNCTIONING

PRESENT LEVEL OF EDUCATiONAL PERFORMANCE: Based on reports from the general education teacher, structured observations made by the ESE teacher, past report cards, and studnet work samples, Emile is able to use the content knowledge and skills he has mastered to benefit from instruction in the fifth grade general education classroom. He turns in approximately 65 percent of the assigned classwork.

As a result of her disability, Emile has difficulty keeping track of and turning in his assignments.

Priority Educational Need: to keep track of and turn in assignments

MEASURABLE ANNUAL GOAL: When completing an assignment, Emile will use various strategies such as using color coded folders for completed work and checklists to keep track of and turn in assignments independently 90% of the given opportunities over one year period.

Annual Goal Evaluation Plan: Progress toward annual goals will be evaluated every grading period by the ESE teacher and general education teacher through analysis of data collected weekly of the number of assignments Emile turns in independently.
